Reporting to the Program Manager, the Care Manager (CM) provides operational and clinical leadership within the Child & Adolescent Tertiary Mental Health Program at the Glenrose Rehabilitation Hospital. The position oversees Day Programming, Allied Health, Regularized Relief, and Casual Relief teams and supports the delivery of specialized mental health services for children, adolescents and their families. The CM is accountable for the effective operation of assigned services, ensuring safe, high-quality, patient- and family-centered care. The incumbent leads multidisciplinary teams, promotes collaboration, accountability, psychological safety and continuous quality improvement while supporting a positive and respectful workplace culture. Responsibilities include workforce planning, staffing, recruitment, scheduling, performance management, staff development, resource allocation and budget oversight. The CM provides coaching and mentorship to support staff engagement, professional growth and service excellence. The incumbent leads quality improvement, change management, and program development initiatives, using performance data and stakeholder feedback to enhance service delivery and patient outcomes. The role supports the implementation of strategic and operational priorities and fosters team capacity to adapt to change. Working collaboratively with physicians, leaders, staff, community partners, and other stakeholders, the CM develops and maintains effective relationships that support coordinated care. The incumbent promotes evidence-informed, trauma-informed, and equitable practices while ensuring compliance with organizational policies, professional standards and legislative requirements. Strong leadership, communication, problem-solving and decision-making skills are essential to achieving program goals and supporting high-performing interdisciplinary teams.

Additional Required Qualifications:
Current registration and good standing with the applicable professional regulatory college where required. Previous management experience in a progressive environment is required. Experience leading multidisciplinary teams and managing clinical and operational activities in a complex healthcare environment.
Preferred Qualifications:
Master’s degree in health services (Clinical Psychology, Social Work, Occupational Therapy, Nursing or related field.) Leadership experience in child and adolescent mental health, rehabilitation or tertiary services. Proven partnership-building across community, education and healthcare sectors. Quality improvement, project or change management experience an asset. Knowledge of family-centered, trauma-informed, evidence-based care, workforce planning, budgeting, and performance management.
