Your Opportunity:

If you ever felt you wanted to do more for people living with Chronic pain and their families, this would be of interest to you. As part of the Alberta Virtual Pain Program (AVPP), this is an opportunity to provide support and education to Albertans living with chronic pain. This position will be responsible for the co-development, coordination, and delivery of chronic pain patient and provider education and self-management tools and strategies through virtual group-based classes, webinars, and other online resources.

Description:

As a Social Worker II, you will be a member of an inter-disciplinary team and provides a range of social work services to patients and their families. Your services may include: psychosocial assessment, resource-based interventions, individual, group and family treatment, risk screening, counselling, case management and coordination, transition or discharge planning, and prevention, promotion and public awareness activities.

Required Qualifications:

Completion of bachelor's degree in Social Work. Active or eligible for registration with the Alberta College of Social Workers (ACSW).

Additional Required Qualifications:

Recent experience with virtual chronic pain management. A minimum of 5 years’ experience providing frontline social work care in a variety of healthcare settings (e.g., acute, community, tertiary). Previous clinical experience in chronic pain management and working with patients living with chronic pain (minimum of 5 years). Excellent computer skills, including working knowledge of Connect Care, Zoom, Microsoft Office applications, and the ability to learn and utilize multiple electronic platforms concurrently. Virtual facilitation experience for groups of patients living with complex chronic conditions. Excellent interpersonal skills and ability to effectively liaise with various professionals. Ability to work confidently, independently and within collaborative partnerships.
