Your Opportunity:
The Social Worker I is an essential member of the integrated recovery-orientated team that works in collaboration with individuals within our correctional health services centers and external stakeholders to facilitate transition into the community. This role supports individuals experiencing mental health and/or substance use challenges by promoting recovery, resilience, and continuity of care as individuals transition from custody to community-based supports. Working collaboratively with interdisciplinary team members and external service providers, the Social Worker I contributes to the planning and delivery of client-centered interventions that complement clinical and outreach services. Description:
As a Social Worker I, you will be a member of a multi- disciplinary team that provides mental health services to individuals, families and/or groups at a community based facility. You will be responsible for the initial screening of patients by gathering and documenting client information, basic assessment and providing support to clients and their families' short-term needs. You will liaise with and refer clients to appropriate community and government services and support, along with participating in case conferences and team meetings.

Completion of diploma in Social Work. Active or eligible for registration with the Alberta College of Social Workers (ACSW).
Additional Required Qualifications:Demonstrated experience in accessing and coordinating housing programs is required. Proven experience working with marginalized populations is essential. The successful candidate will have a strong record of effective collaboration within interdisciplinary team settings. Experience in program development and the ability to establish and maintain productive relationships with community partners are considered assets. Experience working in a health care environment is also an asset. The candidate must demonstrate strong critical thinking and problem-solving abilities, along with excellent communication skills to collaborate effectively with health care team members, patients, and their families in a fast-paced environment. Sound professional judgment, initiative, and effective time management skills are essential.
