Social Worker II

The Social Worker II is an essential member of the integrated recovery-orientated team that works in collaboration with individuals within our correctional health services centers and external stakeholders to facilitate transition into the community. The candidate will be a highly motivated, confident, and dependable individual who collaborates with team members, patients and their families in a fast-paced environment ensuring trauma-informed, equitable care. The candidate will hold working knowledge of health care systems and community resources to support psychosocial interventions such as crisis intervention, resource navigation, advocacy, and referring to internal/external agencies. As a Social Worker II you are part of a team dedicated to providing quality patient centered care through best practices and evidence informed approaches, aligning with social work Code of Ethics.
Description:
As a Social Worker II, you will be a member of an inter-disciplinary team and provides a range of social work services to patients and their families. Your services may include: psychosocial assessment, resource-based interventions, individual, group and family treatment, risk screening, counselling, case management and coordination, transition or discharge planning, and prevention, promotion and public awareness activities.

Completion of bachelor's degree in Social Work. Active or eligible for registration with the Alberta College of Social Workers (ACSW).
